This form is required for all students needing medication during school hours, and is kept on file in the school office.
Child Care Medication Authorization Form (ELC)
This form is required for all students enrolled in the SPCS Early Learning Center needing medication during school hours, and is kept on file with the student’s lead teacher.
Sports Physical
This form is required for all students who wish to participate in SPCS Athletics. Students may not participate in athletic activities without a physical on file. Physicals are good for two years and are kept on file in the school office.
